Sacroiliac Joint Dysfunction

Approximately 80% of adults will have at least one episode of low back pain, many will have repeated or chronic pain. Although this can be caused by problems with the spine and the muscles that attach to the spine a common cause of back pain is sacroiliac joint dysfunction.

The sacroiliac joints join the pelvis to the spine, you have one either side of the sacrum. A very strong joint with small amounts of movement they do nonetheless cause an immense amount of problems if they are injured. Some doctors believe these joints don't move at all but the majority are aware of new research which shows they do.

Some of the symptoms associated with sacroiliac joint dysfunction are:

Low back pain - typically this will affect the side of the spine but if both sacroiliac (SI) joints are affected it will be across the entire low back.

Buttock pain - the SI joints are partly beneath the buttocks and a common area to feel the worst pain is the S3 section of the sacrum which is in the top of the buttock. The buttock pain can be a result of referred pain or on-site muscular dysfunction as a result of an unstable pelvis.

Upper back pain - due to the attachment of the thoracolumbar fascia on the thorax and the sacrum this can cause stiffness in the upper back.

Headaches - if the spine is misaligned it will be affected right to the neck.

Iliotibial band pain - this band runs down the lateral thigh from the hip to the knee and can get distorted, tight or overworked when the pelvis isn't functioning correctly.

Hamstring pain - usually because of tightness. The hamstring attempts to stop the unstable pelvis from moving, tightens and becomes prone to injury.

Groin pain - sometimes referred and sometimes a strained muscle.

This list just goes on and on.

 

The pelvis is the foundation for stability and any assymetrical movements associated with instability can cause a wide selection of painful problems.

The joints may be injured in a twisting, bending or kicking action, or they may just become unstable due to poor muscle control. One side of the pelvis can get stuck in one position and refuse to move even the small amount it is supposed to, this requires a manipulation to mobilise the joint then the exercises to prevent a recurrence. The ligaments become stretched and weak, sometimes painful too, when the joint is either stuck or unstable. The muscles need to be strong enough to hold the joints in their correct position to allow these ligaments to heal or re-injury is likely.

This painful problem responds well to exercise but only if you are exercising correctly. The wrong exercises can set you up for extra misery, further pain and problems and a long recovery time. This is where our programme differs from many. Usually patients are given a standard exercise sheet that targets some of the muscles. For some people this will work, percentages dictate that some people will have the muscle imbalances that these sheets fix. Many will get a small amount of relief compared to the effort they put into exercising which is frustrating to say the least. And some people will get no benefit from these standard exercises at all. Then there will be the people who actually become worse from these because the exercises increase their own individual imbalances.

This is our point. Everyone is an individual; you walk, sit and stand differently to others, your imbalances are brought about by how you move every day so they will be the same as some people's but not everyone's. Our programme addresses this.
